Chronic (Well)ness Symptom Journal
Hey there! Thanks for stopping by. My name is Wolfe Erikson, and like you, I was fed up with doctor's having no idea how to explain the symptoms that I was experiencing. Determined to find a diagnosis and receive appropriate care, I developed this journal to help me recognize patterns in my health. After a month of tracking this data, I was able to take this information to a practitioner, receive an appropriate diagnosis, and get me on a path to recovery.
The path to recovery is an individual experience. Diagnosis can be life-changing, but pursuing it through doctor's appointments alone can be immensely costly, both financially, and emotionally. This journal is a way to help you stay organized and to make the most of your visits with your practitioner. My suggestion is to print 30 copies of this journal, track your symptoms for those 30 days, and use that in your next doctor's appointment.
About the Versions
Upon purchase, you will receive BOTH versions – so don't worry about needing to decide which is best. In one version of the journal, we've provided a baseline of symptoms that are helpful to track if you experience chronic illness. If you have specific symptoms that are not reflected in this list, please use the alternate version with that section open for you to fill in.
Fill In Your Own Symptoms
Alternatively, if you don't see your symptoms reflected in this list, or know that you want to track something else, I've made this alternate version for you.
